VOLUNTEER WAIVER

I, the undersigned hereby acknowledge that I have voluntarily applied to participate in Truckee Litter Corps Street Cleanup Events performing certain services for the Solid Waste and Recycling Division, Town of Truckee.  I understand and agree that a volunteer position does not constitute an employee – employer relationship with the Town and that I will not be reimbursed for any expenses.  I am voluntarily participating in these activities with the knowledge that there is some risk that I may be injured in the course of performing these services.  I understand that the Town’s policy is to cover volunteers as employees of the Town for purposes of Workers’ Compensation benefits.  I also understand that under Workers’ Compensation laws, Workers’ Compensation benefits will be the sole and exclusive remedy in the event I am injured while performing these volunteer activities and services.  I have no medical limitations which preclude me from serving as a volunteer for the Town.

With the exception of Workers’ Compensation benefits as set out above. I hereby agree that I, my heirs, guardians, legal representatives and assigns will not make a claim against or file an action against the Town of Truckee or any of its agents, officers or employees, for injury or damage resulting from negligence, howsoever caused, by any employees, agent or officer of the Town of Truckee as a result of my participation in this volunteer activity or service. In addition, I hereby release and discharge the Town of Truckee, its agents, officers, and employees from all actions, claims and demands that I, my heirs, guardians, legal representatives or assigns now have or may hereafter have for injury or damage resulting from my participation in these volunteer activities or services.

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A PARTIAL RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND THE TOWN OF TRUCKEE AND SIGN IT ON MY OWN FREE WILL AND AFTER THE OPPORTUNITY TO HAVE IT REVIEWED BY AN ATTORNEY OF MY CHOICE

Feed Truckee Release of Liability Waiver

I, the undersigned hereby acknowledge that I have voluntarily applied to participate in Feed Truckee’s food rescue initiative as a food delivery driver for the Solid Waste and Recycling Division, Town of Truckee. I understand and agree that a volunteer position does not constitute an employee – employer relationship with the Town and that I will not be reimbursed for any expenses. I am voluntarily participating in activities that will require the lifting and carrying of up to 50 pounds with the knowledge that there is some risk that I may be injured in the course of performing these services. I understand that the Town’s policy is to cover volunteers as employees of the Town for purposes of Workers’ Compensation benefits. I also understand that under Workers’ Compensation laws, Workers’ Compensation benefits will be the sole and exclusive remedy in the event I am injured while performing these volunteer activities and services. I have no medical limitations which preclude me from serving as a volunteer for the Town. I certify that I have a valid driver’s license and valid car insurance, and I agree to deliver donated food directly to Sierra Community House after pick-up, without delay.
